Follow these directions if?

You are unable to locate your current valid passport.
Your passport has been stolen, and it is still valid.

Complete and gather the following items listed below

1. Fill out Application for New Passport (DS-11)

Make sure you DO NOT sign the DS-11 form until you are instructed to do so, refer to item # 9

2. Fill out Statement of Lost or Stolen Passport (DS-64)

Make sure you sign the form.

When completing Form DS-64, do so legibly and in black ink. Answer all questions completely. If you do not know the answer in detail, be as exact as possible.

If you are not applying for a replacement passport at this time, but wish to report the loss or theft of your U.S. passport, submit Form DS-64 to:

3. Proof of Citizenship

You will need to have in your possession one of the following documents:
An original State Issued or certified copy of U.S. Birth Certificate
A current or expired U.S. Passport
Original Naturalization Certificate

Please note: Notarized copies are not acceptable, provide original documents only.

4. Passport Photographs

Two (2) identical recent passport size (2 in X 2 in) photos
** Photographs must be in color on a white background. Ensure you are not wearing any head covering. (If head covering is worn, you must submit in writing the reason why you are wearing a covering i.e. religious reasons. Document must contain your original signature.) **

5. Proof of Identification

You must have in your possession:
A valid state issued driver’s license, identification card or military I.D bearing a photo.
Identification must be at least 6 months old. If the Identification presented is not six months old you will need to provide supplemental forms of your identity acceptable by the U.S. Dept of State.

6. Proof of Departure

You must provide one of the following items:
A copy of your airline ticket (2 copies)
A copy of your travel itinerary from a travel agent or online agency (such as Orbitz or Expedia) (2 copies)
** If this is a business trip and you cannot provide a confirmed itinerary, you will need to submit a letter on company letter head explaining the purpose of your trip and expected dates of travel. **

7. Release Letter/Letter of Authorization

Fill out and sign two (2) copies of the Release letter/Letter of authorization giving A1 Passport & Visa, LLC permission to process the passport on your behalf.

8. Government Fee and Service Fee

You will need to have two (2) checks or money orders
One check or money order is payable to U.S. DEPARTMENT OF STATE in the amount of $170.00
(This is the government expedite processing fee)

Make the second check or money order in the amount of $25.00 payable to the Post Master or County Clerk, refer to step # 8. (Cash also accepted)

Note:
1. Temporary checks are not acceptable
2. Your name and address must be printed on top of the check, not handwritten (Company checks are acceptable)
3. The applicant's name and date of birth must appear in the memo section of the check.

9. Local Acceptance Agencies

The U.S Department of State requires that all applicants applying for a new passport have their applications verified and witnessed by an authorized agent at a local post office or county clerk. This agent will instruct you to sign the DS-11 application after administering you the oath.

You must have the following documents in your possession when going to the Acceptance Agency:
  •   DS-11 form filled out, but not signed
  •   DS-64 form filled out, ensure you sign the form
  •   Proof of Citizenship
  •   Two passport photographs
  •   Valid Drivers License or supplemental identification
  •   Proof of Departure
  •   Letter of Release/Authorization
  •   Check or money order in the amount of $170.00 made out to “U.S. Department of State”
  •   Check, Cash or Money Order in the amount of $25.00 for the Acceptance agency

After paperwork is verified you will be asked to sign the application.
The documents will then be sealed in an envelope and returned to you. This sealed envelope will need to be delivered to our office to begin the processing of your passport.
 
** Please Note: If all of the following documents are not enclosed in the sealed package, you will experience delays in the processing of your passport: **
DS-11 form fully executed · Proof of Citizenship · Two passport photographs · Valid Drivers License · Proof of Departure · Letter of Release/Authorization · Check or money order made out to “U.S. Department of State” in the amount of $170.00 for an Adult or $140.00 for a Minor

10. Fill out the Information Sheet, Pay Fees On-line and Hand Deliver or Mail your documents to our office.

PAY ONLINE

Choose the “PAY ONLINE” tab and select your service from the menu of products, add them to the cart and “check out”. You will then be directed to create an account. Your passport will be returned to you at the address listed and based on the method of shipping chosen at the checkout menu.

Checks should be made payable to: A1 Passport & Visa, LLC

In the memo section of the check the applicants name and date of birth should appear.

Two business days before my husband and I were leaving to take our daughter to Europe as a high school graduation present, we discovered that my husband's passport had expired.
