State Department requirement for persons under the age of 16 years:

• Must apply in person with both parents/guardian(s).
• Do not sign the application until instructed to do so by the Acceptance Agent.
• Must provide his/her Social Security number.
• All children regardless of age, including newborns and infants, must have their own passport.
• There are special requirements for All Minors Ages 16 & 17

Complete and gather the following items listed below

1. Fill out Application for New Passport (DS-11)

Make sure you DO NOT sign it until you are instructed to do so, refer to item # 8

2. Proof of Citizenship

You will need to have in your possession one of the following documents:
An original State Issued or certified copy of U.S. Birth Certificate
A current or expired U.S. Passport
Original Naturalization Certificate

Please note: Notarized copies are not acceptable, provide original documents only.

3. Passport Photographs

Two (2) identical recent passport size (2 in X 2 in) photos
** Photographs must be in color on a white background. Ensure you are not wearing any head covering. (If head covering is worn, you must submit in writing the reason why you are wearing a covering i.e. religious reasons. Document must contain your original signature.) **

4. Proof of Identification

Both parents must have in their possession:
A valid state issued driver’s license, identification card or military I.D bearing a photo.
Identification must be at least 6 months old. If the Identification presented is not six months old you will need to provide supplemental forms of your identity acceptable by the U.S. Dept of State.

If applicant is age 16 or 17 years, only one parent need to appear and the applicant should hold a valid school identification, state issued drivers license or identification card.

5. Proof of Departure

You must provide one of the following items:
A copy of your airline ticket (2 copies)
A copy of your travel itinerary from a travel agent or online agency (such as Orbitz or Expedia) (2 copies)
** If this is a business trip and you cannot provide a confirmed itinerary, you will need to submit a letter on company letter head explaining the purpose of your trip and expected dates of travel. **

6. Release Letter/Letter of Authorization

Fill out and sign two (2) copies of the Release letter/Letter of authorization giving A1 Passport & Visa, LLC permission to process the passport on your behalf.

You will need to have two (2) checks or money orders
One check or money order is payable to U.S. DEPARTMENT OF STATE in the amount of $140.00
(This is the government expedite processing fee)

Make the second check or money order in the amount of $25.00 payable to the Post Master or County Clerk, refer to step # 8. (Cash also accepted)

Note:
1. Temporary checks are not acceptable
2. Your name and address must be printed on top of the check, not handwritten (Company checks are acceptable)
3. The applicant's name and date of birth must appear in the memo section of the check.

The U.S Department of State requires that all applicants applying for a new passport have their applications verified and witnessed by an authorized agent at a local post office or county clerk. This agent will instruct you to sign the DS-11 application after administering you the oath.

If one parent is unable to appear, the absent parent must complete the DS-3053 Child Consent Form and have it notarized

After paperwork is verified you will be asked to sign the application.
The documents will then be sealed in an envelope and returned to you. This sealed envelope will need to be delivered to our office to begin the processing of your passport.

9. Fill out the Information Sheet, Mail Envelope and Pay Online

Click here to download the Information Sheet, fill it out and send it along with your sealed envelope. ** If this is a new passport application, ensure that one copy of the Release/Authorization Letter is on the outside of the sealed envelope. **

PAY ONLINE

Choose the “PAY ONLINE” tab and select your service from the menu of products, add them to the cart and “check out”. You will then be directed to create an account. Your passport will be returned to you at the address listed and based on the method of shipping chosen at the checkout menu.

I highly recommend this company to everyone. I was informed of a business trip to Mexico I only had two days to get my passport so I called Omar in Houston and he informed me I do a 24 hr passport service the sealed pkg was to be there at 8:00 am but UPS was late so the passport agency could not do the same day passport. The day I was leaving from Oklahoma city Omar hand delivered my passport to southwest airlines in Houston to have it delivered in Okc before I left. Cause of Omar I made it to Mexico and kept my job.
