You can use this option if?

You need to add visa pages to your valid U.S. passport book.
You have 12 months or more remaining on your current passport.

Complete and gather the following items listed below

1. Fill out Application for Additional Visa Pages (DS-4085)

Make sure you sign the form

2. Passport

You will need to provide us with your valid U.S. Passport.

3. Proof of Departure

You must provide one of the following items:
A copy of your airline ticket (2 copies)
A copy of your travel itinerary from a travel agent or online agency (such as Orbitz or Expedia) (2 copies)
** If this is a business trip and you cannot provide a confirmed itinerary, you will need to submit a letter on company letter head explaining the purpose of your trip and expected dates of travel. **

4. Release Letter/Letter of Authorization

Fill out and sign two (2) copies of the Release letter/Letter of authorization giving A1 Passport & Visa, LLC permission to process the passport on your behalf.

5. Government Fee and Service Fee

You will need to have two (2) checks or money orders
One check or money order is payable to U.S. DEPARTMENT OF STATE in the amount of $142.00
(This is the government expedite processing fee)

Note:
1. Temporary checks are not acceptable
2. Your name and address must be printed on top of the check, not handwritten (Company checks are acceptable)
3. The applicant's name and date of birth must appear in the memo section of the check.

Pay On-Line and Ship Your Documents

ENSURE YOU HAVE IN YOUR POSSESSION:
• DS-4085 form filled out and signed
• Valid U.S. Passport
• Proof of Departure
• Letter of Release/Authorization
• Check or money order in the amount of $142.00 made out to “U.S. Department of State”

6. Fill out the Information Sheet, Pay Fees On-line and Hand Deliver or Mail your documents to our office.

PAY ONLINE

Choose the “PAY ONLINE” tab and select your service from the menu of products, add them to the cart and “check out”. You will then be directed to create an account. Your passport will be returned to you at the address listed and based on the method of shipping chosen at the checkout menu.

In the memo section of the check the applicants name and date of birth should appear.

Omar and A1 Passport and Visa to the rescue! I was checking in with a group at British Airways in Houston when I was told that my passport had expired. I felt devestated! A friend in the group googled passport services on her cell phone and found Omar's business and phone number but it was late afternoon and we figured we'd have o wait till the next day to see him. My husband and I checked into a hotel and then took a chance and called Omar's number to find out when we could see him.
